Work Permits for School Age Students. One is for students under 16 years of age and one is for students 16 and older. The top of the form is to be completed by the student and their parent. The form is then to be taken to the student's employer to complete the middle section of the form. Finally, the form is brought to the school to receive approval by school personnel. A copy will be kept on file in the student's permanent record and the original will be returned to the employer to be kept while the student is in their employ.
